Spill or Unauthorized Discharge Notification Form

To protect and preserve the region's primary groundwater supply, the EAA requires notification of discharge or spills of oil, petroleum products, used oils, hazardous substances, industrial waste, or other substances on the Recharge Zone or Contributing Zone of the Aquifer.

Notification requirement can be found in chapter 713, Subchapter E (Spill Reporting) of the EAA Rules
