When submitting your request, please include the employee’s/former employee’s signature that authorizes the employer to release information.
Business hours are Monday – Friday, 8 a.m. – 5 p.m.
Expected Turnaround Time
Please allow 1-3 business days for your request to be completed. Verification involving Wage Loss/Disability claims may take 1-5 business days.
